SCLEROTHERAPY

What is Sclerotherapy?
Sclerotherapy is a medical procedure used to treat varicose veins and "spider veins." Sclerotherapy involves injection of a solution (Dr. Malik uses sodium chloride) directly into the varicose vein. The solution irritates the lining of the vessel, causing it to swell and stick together, and the blood to clot. Over time, the vessel turns into scar tissue that fades from view. Sclerotherapy is a well-proven procedure has been in use since the 1930s to treat varicose veins.

How is Sclerotherapy done?
Sclerotherapy is performed in our office. In most cases, a 23 percent sodium chloride sterile solution with or without addition of Lidocaine is injected directly into the blood vessel, using a very fine needle. The number of varicose veins injected in one session is variable, depending on the size and location of the veins, and the patient’s overall medical condition. Mild disconfort may occur with leg cramping for 2 to 3 days. Usually Tyelenol is sufficient for discomfort and no prescription pain medication is needed.

How successful is Sclerotherapy in treating varicose and spider veins?
Sclerotherapy works well for most patients with varicose veins. It is estimated that as many as 50 percent to 80 percent of injected varicose veins may be eliminated with each injection session. A few (less than 10 percent) of the people who have Sclerotherapy on their varicose veins do not respond to the injections at all. In these instances, a different method, such as laser therapy or surgical vein stripping, may be tried. In general, spider veins respond to Sclerotherapy in 3 to 6 weeks, and larger veins respond in 3 to 4 months. If the veins respond to the treatment, they will not reappear. However, new veins may appear over time. If needed, you may return for injections.

What do I need to do before the procedure?

Prior to Sclerotherapy, certain medications should be avoided.Tetracycline or Minocin, both antibiotics, may possibly cause a staining of the skin if taken 7 to 10 days before or after Sclerotherapy. Ask the doctor about other antibiotic medications you may take, or ask for safe guidelines for discontinuing these medications. If you are required to take an antibiotic before any invasive procedure, such as dental procedures, colonoscopy or surgery, please inform Dr. Malik .

Do not take aspirin, ibuprofen or other NSAIDS. (i.e. Advil and Nuprin) or other anti-inflammatory medications for 48 hours before and after Sclerotherapy, because these medications may interfere with the action of the sclerosing agent. Tylenol is permitted. Ask your doctor for specific guidelines before discontinuing any medication prior to Sclerotherapy.

Prednisone also decreases the effectiveness of the sclerosing agent. Ask the doctor who prescribed your prednisone if it can be safely discontinued for 48 hours before the Sclerotherapy procedure.

No lotion is applied to the legs before or after Sclerotherapy. It is recommended that you bring a pair of shorts to wear during the varicose vein treatment procedure.

If you have compression hosiery from previous treatments, please bring them with you so we can make sure they will provide adequate support after the Sclerotherapy procedure.

What are the common side effects of Sclerotherapy?
Larger injected varicose veins may become lumpy and hard for several months before resolving. Raised red areas may appear at the injection sites and should disappear within a few days. Brown lines or spots on the skin may be noted at the site of the injection, possibly caused by a form of iron that escapes from the injected veins. In most cases, they disappear within 3 to 6 months, but can be permanent about 5 percent of the time. Bruising may occur around the injection site and can last several days or weeks. There is a common risk of developing a network of fine red blood vessels following injection which may resolve on its own within a few months or can be treated with further injections.

Rare Risks after Sclerotherapy. There is a small possibility of Deep Venous thrombosis. This risk is higher in those predisposed to blood clotting through inherited Thrombophilia, lack of exercise, air travel, following major operations, and women taking birth control pills or HRT. There is risk of ulceration at injection site, which will eventually heal but may leave a scar.

How will I know if I am a candidate for Sclerotherapy?
Prior to the procedure, you will have an initial consultation with Dr. Malik to determine if you are eligible for Sclerotherapy. You are not eligible for Sclerotherapy if you are pregnant, breastfeeding, or are bedridden. You must wait at least three months after birth before you can be considered for this procedure. You are considered high risk for formation of blood clots after Sclerotherapy if you take birth control pills or If you have had a blood clot in the past. Dr. Malik does not offer Sclerotherapy to patients currently on birth control pills, or on HRT and to patients with history of blood clots. veins that are potentially usable for future surgical bypass procedures (such as the saphenous vein for coronary artery bypass graft surgery) will generally not be considered for injection, unless they are already deemed unusable for bypass procedures.

What is Lipodissolve?
Lipodissolve is a non surgical procedure for localized fat reduction. A mixture of soy derived formula causes breakdown of fat in the area injected, which in turn is excreted through kidneys.

Who is a candidate for Lipodissolve?
Lipodissolve is suitable for non-obese patients with localized fat deposits which cannot be reduced with appropriate diet and regular exercise. Lipodissolve does not result in weight loss. It is used to reduce fat under chin, abdomen, love handles, buffalo hump, thigh saddlebags, wings (area on the back of armpits), and other areas of localized fat deposits.

Who is not a candidate for Lipodissolve?
1. Pregnant and breast feeding mothers.

4. Serious obesity or unrealistic expectations.

What are the side effects of Lipodissolve?
Side effects are mostly localized at the injection site and transient. The side effects of Lipodissolve procedure are:

1. Pain, mild to moderate, Itching, redness and tenderness may last 3 to 7 days.
2. Swelling, mild to moderate lasting 3 to 7 days. This is especially worse at soft tissue sites such neck or lower eyelids.
3. Possibility of infection at injection site.

What to do after the injection?
1. Apply ice to the injected site for first 48 to 72 hours.
2. We suggest taking Over the counter anti histamine such as Claritin or Zyrtec to minimize,swelling, itching and redness.
3. We recommend Ibuprofen or Acetaminophen for pain. Prescription pain medication is availble to patients to minimize post injection discomfort if needed.
4. Most Patients resume their normal daily activities the same day.

How soon will I see results?

Most people need 6 to 10 sessions. Sessions are done at least 3 weeks apart.

Most people start seeing some results 10 to 14 days after the first treatment. Results are cumulative and fat reduction continues for several weeks after each session. call our office for a complimentary consultation. For optimum results it is recommended that you combine your treatments with a healthy lifestyle consisting of:

• Minimum 20 to 30 minutes of aerobic activity each day.
• Diet low in high glycemic carbohydrates and saturated fats.
• Adequate intake of protein, multivitamins and lots of water.

How many sessions do I need?

• We start treatment in most patients ith 6 sessions .
• During each session 2 to 4 areas can be treated depending on the size of each area.

Photodynamic Therapy (PDT) With Levulan (ALA)

Photodynamic Therapy (PDT) With Levulan is a treatment that combines light and a special light-activated topical photosensitizing agent called Levulan. Levulan is a 20% solution of Aminolevulinic Acid (ALA). ALA occurs in our body as a precursor for hemoglobin synthesis and is required to make our red blood cells. As such it is a natural compound.

Upon application to skin, Levulan is absorbed by actinic keratosis cells and converted into a substance making these cells highly sensitive to specific light. The skin is then exposed to Intense Pulse light which destroys damaged skin cells.

Photodynamic treatment removes sun damaged pre-cancerous zones and actinic keratoses. Skin texture, fine lines, pores, Acne, sebaceous cyst size and blotchy pigmentation are also improved with the treatment.

What conditions can be treated with ALA/PDT Therapy?

  • Actinic keratosis
  • Prevention of recurrent Actinic keratosis in susceptible patients
  • improvement of superficial wrinkles
  • Reduction of pore size
  • Rough skin texture
  • Superficial brown solar spots
  • Management of moderate to severe cystic acne

How many treatments will it I need?

·  For Actinic Keratosis, most patients have complete clearance of skin lesions with 2 to 4 treatments, 4 weeks apart. Because AK is considered an early squamous cell cancer, we like to be deiligent with close follow up and complete eradication of all lesions..

·   For Cystic Acne, a series of 3 to 6 treatments may be needed to control formation of new cysts.

· Follow up or maintenance treatments are done depending on individual skin type and rate of recurrance of any skin conditions.

What are treatments like?

· Upon arrival to the office, your skin is washed with Cetaphil followed by application of Levulan The solution is left on your skin for 15-60 minutes.

You may be required to take prescribed pain medication to reduce discomfort at this time.

· If you have history of recurrent cold sores, you may be required to take an antiviral mediation.

· The skin is washed thoroughly with Cetaphil to wash off all Levulan solution.

· Your skin is exposed to Intense Pulse light which takes about 15 to 20 minutes.

Following ALA/PDT, the treated areas can appear red with some peeling for 2-7 days. Some patients have an exuberant response to PDT, and experience marked redness of their skin. Temporary swelling of the lips and around your eyes can occur for a few days. Darker pigmented patches called liver spots can become temporarily darker and then peel off over 7 to 10 days leaving normal skin.

How much improvement can I expect?

Patients with severe sun damaged skin manifested by actinic keratosis, texture, and tone changes including mottled pigmentation and skin laxity may see excellent results. You may also see improvements of large pores and pitted acne scars. Active acne can improve dramatically.

What do I need to do after the treatment?

· It is absolutely essential to avoid direct exposure to sun for at least 48 hours (stay indoors) after the treatment. Severe sun burns have been reported with sun exposure following treatment.

· Wear at least SPF 30 at all times even if you are indoors.

· Use gentle cleanser (Cetaphil) to wash treated area twice a day and apply a bland moisturizer (our Intense Moisture or Aquaphor) liberally until skin is completely healed.

If you have Actinic keratosis, extensive sun damage or severe cystic Acne, you might be a candidate for PDT. Call our office for a complementary consultation with Dr. Malik to dtermine if this will be the right treatment for you.
